No Surprises Act / Good Faith Estimate

At Wallick Nutrition Counseling, LLC (WNC), we value clarity and calm around money. If you are uninsured or not using insurance (self-pay), you have the right to receive a Good Faith Estimate (GFE) of expected charges for non-emergency services.

What is a Good Faith Estimate?
A GFE is a written estimate of the total expected cost of your care based on the information we have at the time. It helps you understand fees up front and compare options. A GFE is not a contract and not a guarantee of final charges—your clinician will recommend services according to your needs and clinical judgment. If your plan of care changes, you may request an updated estimate at any time.

When you’ll receive a GFE
  • If you schedule an appointment ≥10 business days in advance, you’ll receive your GFE within 3 business days of scheduling.
  • If you schedule 3–9 business days in advance, you’ll receive your GFE within 1 business day of scheduling.
  • You may also request a GFE before scheduling; we’ll provide it within 3 business days of your request.
  • We also provide updated GFEs annually or when your plan of care materially changes.

What your GFE includes
  • Your name/contact information and our practice details
  • The services reasonably expected for your care (descriptions and, when available, codes)
  • Itemized fees and a total estimated charge for the time period covered
  • A note that actual services may vary and you can request an updated estimate if your plan changes
Not included: charges from other providers or facilities (e.g., outside labs, prescriptions, medical visits) billed separately.

Typical services & fees at WNC (examples; your GFE reflects your specific plan)
  • Initial Nutrition Assessment (60 min): $220
  • Follow-Up Nutrition Session (55 min): $160
  • Letters/Forms & Legal/Administrative Requests: $35 per 15 minutes
  • Late Cancellation/No-Show (per policy): Full session fee

Optional care examples (for context)
  • Month example: 1 intake + 3 follow-ups = $220 + (3×$160) = $700
  • Quarter example: 1 intake + 8 follow-ups = $220 + (8×$160) = $1,500

(Telehealth is billed at the same rate.)

If your bill is higher than your estimate
If you receive a bill that is $400 or more above your GFE for the same services/time period, you may initiate the Patient-Provider Dispute Resolution (PPDR) process with HHS within 120 days of the bill date. (HHS charges a $25 administrative fee to begin the PPDR.)
